Filler Up: Dallas Company Offers a Non-Surgical, 'Down There' Procedure For Men

There comes a time in every man’s life when he wonders, “What if it were just a little bigger?”

This thought may come from a locker room glance, or comparisons while watching porn or social media, but whatever the trigger, the thought is almost universal. The problem? Most so-called solutions range from ineffective to downright terrifying.

Penile girth enhancement methods, such as pills, pumps and some surgical procedures, often lack scientific support and can present health risks. The American Urological Association also cautions that some PGE procedures are not proven to be safe or effective.

A Dallas local may have cracked the code on safer penile enhancement. William “Bill” A. Moore, founder of AdvancedYOU, developed PhalloFILL, a non-surgical procedure that uses hyaluronic acid dermal filler, just like lip injections but below the belt.

“I think that PhalloFILL is the best thing that's ever happened to a man as far as male enhancement,” Moore says. “There's not a man that really walks the face of this Earth, if they're going to be honest, that wouldn't take a little bit of a larger penis.”

The PhalloFILL procedure begins with a consultation to assess the patient's goals and determine the appropriate amount of the filler needed for a natural-looking result, though Moore may occasionally allow older patients to choose a different size.

On the day of the procedure, the area is first numbed with a topical anesthetic to minimize discomfort, Moore describes it as a pinch. Then, the derma filler, commonly used in facial aesthetics, is carefully injected into the penile shaft to add volume and increase girth.

The procedure itself typically takes 20 minutes, Moore says, and most patients can return to their normal activities within a day or two but cannot pleasure themselves or engage in sex until after 10 days.

Results are visible immediately with increases in size following each scheduled appointment, Moore says.

Finally, a sleeve called PhalloSleeve is slid onto the penis in order to keep the derma fill proportionate whenever the male organ retracts from erect to flaccid. According to Moore this sleeve “is patented in 44 countries now as a medical device specifically for use as penile for penile growth enhancement.”

Both Moore and Dr. Amy Pearlman, a board-certified urologist with expertise in male sexual health, say that if a patient is dissatisfied or experiences complications, the filler can be dissolved with a hyaluronidase injection, an enzyme that breaks down hyaluronic acid.

“If someone wanted me to dissolve the filler, I can easily do that with an office procedure,” Pearlman says. “They come in, I put a tiny little needle and I can dissolve the filler in real-time. I've never had someone come in and say, ‘I want you to dissolve all my filler,’ but if they need something to be more smooth, I can easily do that in a quick session.”

Moore created this procedure after the Priapus shot, a medication-free procedure used to treat erectile dysfunction that also was thought to increase size. Known as the P-shot, it was not effective and had yet to prove Moore otherwise. But Moore had already sent a newsletter in 2010 advertising the procedure and received a “tremendous response” from the 10,000 recipients of his newsletter.

“That was the first time that we had ever talked about enhancement or enhancing the size of the penis,” Moore says. “And so I knew that there was a need that men wanted it.”

Between 2015 and 2017, Moore first saw dermal filler being used for growth enhancement and saw men entering his clinic seeking his aid for a reversal as he had experience in filling faces. His theory that the procedures can be reversed with a hyaluronidase injection was proven true, giving him the confidence to test out derma filler on volunteers during the months his clinic was closed due to the pandemic.

Men of different ages, cultures and races have gone through the procedure, according to Moore and Dr. Pearlman. Moore says that even men who are already in committed relationships often reach out for the procedure for a change in their sex life.

One 49-year-old man, who did not want his identity disclosed, has been married for 30 years and finished his PhalloFILL treatment in late July. He said he began his treatment when he was 48 because he wanted to “change the dynamics” with his wife sexually.

According to the former patient, he felt only a pinch the first two times but generally did not feel pain from the procedure. He grew about half an inch entirely from a starting point of 5.5 inches and believes it to be safe from his experience.

“When you're committed to one woman for more than a quarter century, it's good to have a change, especially a positive change like that, and it's definitely been good for me,” he says.

Moore says he had one patient in his 40s who had a penis size around 4 to 4.5 inches and had the “thinnest penis” he had ever seen.

The man was successful in his career and conventionally attractive according to Moore, but the man told Moore that he wanted to go through the treatment because women would leave during sex because they were unsatisfied with his size.

“He told me that he had started telling women on the first date at dinner that he had an unusually small penis because he couldn't handle them giving up and walking away during sex anymore,” Moore says.

Getting Pumped

After three out of five treatment days, the man called Moore telling him that he felt satisfied with the treatments and the size he had put on. He thanked him, saying the procedure saved his life and that he finally found himself in a relationship with satisfying sex.

According to a section in The Journal of Sexual Medicine Volume 21 titled “Penile size dissatisfaction: (going beyond) the role of the mental health professional,” 42 to 55% of men are dissatisfied with their penis size.

“There's guys that get up and every morning they look in the mirror and they see an unusually small penis,” Moore says. “And then they go about their day with this black cloud over their head and everything that they do is going to have that energy that was created that morning when they saw themselves in the mirror.”

Men of varying sizes often seek PhalloFILL, according to Moore, with most men being within the normal range of size. Pearlman says some men seek treatment for a change in sensation, which the treatment allows, so that men of different sensitivities can achieve peak stimulation based on their desires.

An abstract conducted by Pearlman examining 500 patients who had the enhancement over a three-year time span found only two patients with infections due to noncompliance with post-treatment care. Three other patients received granulomas, which is a cluster of immune cells and inflammation that builds to isolate foreign substances. Moore says that these are generally very small and can be eliminated with hyaluronidase.

The PhalloFILL procedure, done in the style that Moore created, is taught by Moore only to urologists who meet a criteria and even then, not everyone trained gets approved if they do not meet the skill criteria needed.

There are 30 PhalloFILL locations with two foreign locations: one in Montreal, Canada and another in Mexico City. According to Moore and Pearlman, there is demand for more locations outside of North America, specifically in Europe.

Pearlman believes that the discussion for penis growth enhancement should not be stigmatized (on places like Reddit) and says urologists need to get comfortable talking about the subject now that there is a safer option.

“None of my patients are expecting that a bigger penis will solve all their problems, but we identify their concerns and fortunately these days we now have a safe haven that can easily address penile size,” Pearlman says.
